چکیده

These notes detail my 40-minute lecture given 11 June 1995 as part of the Seventh Annual Intensive Review of Internal Medicine, a symposium conducted by the Cleveland Clinic. I was asked to review how probability is used with diagnostic testing. Our main question is: How can diagnostic results and other information about a patient be used to estimate the chance that he/she has a particular “disease?” First, however, we must address: In general, how well does the test perform as a diagnostic tool for this disease?

Probabilistic analysis of global performances of diagnostic tests: interpreting the Lorenz curve-based summary measures.

Several indices based on the receiver operating characteristic curve (ROC curve) have previously been found to possess probabilistic interpretations. However, these interpretations are based on some unrealistic diagnostic scenarios. In this paper, the author presents a new approach using the Lorenz curve. The author found that the summary indices of the Lorenz curve, that is, the Pietra index a...

Significance Testing as Perverse Probabilistic Reason- ing: Supplemental Material

Fundamental ‘forward’ and ‘backward’ probabilities in diagnostic testing Interpreting diagnostic tests When a patient ‘tests positive’ for a disease, what is the probability that this patient actually has the disease? For concreteness, consider using the serum level of brain natriuretic peptide (BNP) as a test for congestive heart failure, defining a positive result as BNP > 100, which reported...

Diagnosis: making the best use of medical data.

To take the best possible care of patients, physicians must understand the basic principles of diagnostic test interpretation. Pretest probability is an important factor in interpreting test results. Some tests are useful for ruling in disease when positive or ruling out disease when negative, but not necessarily both. Many tests are of little value for diagnosing disease, and tests should be o...

Diagnostic Value of Non-stress Test Interpreted by Smart Interpretive Software

Background and aim: Using appropriate methods for the assessment of fetal health including non-stress test during high-risk pregnancies due to possible placental insufficiency is of paramount importance. Due to complexity in medical decisions, using information systems is being increased to support complex medical decisions. We conducted this study to measure the diagnostic value of non-stress ...
